بررسی غلظت پلاسمایی پپتید ناتریورتیک دهلیزی در اسب‌های مبتلا به پس‌زدگی دریچه ریوی قلب

چکیده

پپتید ناتریورتیک دهلیزی (atrial natriuretic peptide; ANp < /strong>) توسط سلول­های عضلانی قلب به­دنبال افزایش حجم خون و کشیدگی دیواره حفرات قلب، ترشح می­شود. افزایش غلظتANp < /strong> در بیماری­های قلبی انسان و اختلالات دریچه­ای سمت چپ قلب اسب مشاهده شده ­­است. هدف از مطالعه حاضر، مقایسه غلظت پلاسماییANp < /strong> در اسب­های سالم و اسب­های مبتلا به پس­زدگی دریچه ریوی و بررسی ارتباط آن با پارامترهای اکوکاردیوگرافی بود. بدین منظور با انجام معاینات بالینی و اکوکاردیوگرافی بر روی تعدادی از اسب­های پرش در اسب­داری­های اطراف تهران، تعداد 11 رأس اسب مبتلا به پس­زدگی دریچه ریوی شناسایی شده و در گروه 1 و 14 رأس اسب که فاقد نشانه­های بیماری بودند، در گروه 2 قرار داده شدند. پارامترهای اندازه­گیری­شده در اکوکاردیوگرافی دوبعدی، یک­بعدی و داپلر اسب­های مذکور، ثبت و مقایسه گردید. همچنین از سیاهرگ وداج آن­ها، نمونه­های خون جمع­آوری و غلظت پلاسماییANp < /strong> با روش ساندویچ الایزا و با استفاده از کیت اختصاصی ANp < /strong>اسب اندازه­گیری شد. بررسی یافته­ها نشان داد که به جز میانگین قطر داخلی بطن راست، اختلاف معنی­­دار دیگری در پارامترهای اکوکاردیوگرافی دو گروه وجود نداشت. میانگین غلظت پلاسمایی ANp < /strong> در گروه 1 و 2 به ترتیب 87/39±2/6 و 45/2±78/4 پیکوگرم در میلی­لیتر بود ولی این تفاوت از لحاظ آماری معنی‌دار نبود. اما بین غلظت پلاسمایی ANp < /strong> با طول بطن راست و عرض بطن چپ، همبستگی معنی­داری ثبت گردید (05/0 p < /em>) .
نتایج مطالعه حاضر نشان داد که بین غلظت ANp < /strong>و اندازه بطن راست قلب اسب ارتباط مستقیم وجود دارد. از این­رو می­توان انتظار داشت، در بیماری­هایی که موجب اتساع بطن راست قلب اسب می­شوند، غلظت این پپتید افزایش یابد.

چکیده [English]

Atrial natriuretic peptide (ANP) is secreted by cardiac myocytes and its plasma concentration often increases in patients with cardiovascular disease in response to blood volume expansion and stretching of cardiac chambers. Increase in plasma concentration of ANP (Cp < sub>ANp < /sub>) has been observed in humans with cardiac diseases and horses with left-sided valvular disease. This study aimed to investigate plasma ANP level in horses with pulmonic valve regurgitationand its association with echocardiographic parameters. Clinical and echocardiographic examinations were performed on several jumping horses in the equestrian clubs of Tehran, Iran. Eleven horses with pulmonic valve regurgitation were identified by echocardiographic examination and were included in the case group (Group A), and 14 healthy horses were taken as the control group (Group B). Then, B-Mode, M-Mode and doppler echocardiographic parameters were measured and compared between the two groups. Blood samples from the jugular vein were collected in tubes and were transferred to laboratory. Cp < sub>ANp < /sub> was measured by Sandwich ELISA method using an equine specific kit.MeanCp < sub>ANp < /sub>of horses in group­1 and group2 were 6.39±2.87 pg/ml and 4.78±2.45 pg/ml respectively, but no significant differences were observed. Except for right ventricular internal diameter, other echocardiographic parameters were not significantly different between the two groups. Cp < sub>ANp < /sub> was significantly correlated to systolic right ventricle length and left ventricular width. In conclusionno significant differences were observed between Cp < sub>ANp < /sub> of healthy horses and horses with pulmonic regurgitation. Based on the Correlation between CpANP and right ventricle length, an elevation in Cp < sub>ANp < /sub> might be expected in severe pulmonic regurgitation with change in ventricle`s dimensions.
